Nitya Puja Sponsor Enrollment Form
This form to be filled in only by new sponsors.
- GNHT BOD thanks all those that are currently enrolled in this program for you support to the Temple.
- Existing sponsor do not have to fill this form.
- This form is only for devotees who are new to this program and wish to enroll.
What are the incentives for participation in Nitya Puja Sponsorship Program?
Five (5) FREE ARCHANA tickets for each year that you are contributing to the program
- Priest will include all enrolled devotees and their family members added in the enrollment form in the Nitya
- Pooja that is being performed daily at the temple.
How much does it cost?
$1 a day or $365 a year
How frequently should I pay?
On a monthly or Yearly basis, choose one that is convenient for you.
We request you to choose the same date and same amount for each payment period within a year
How do I make the sponsorship payment?
When do I have to start sponsorship payment?
Payment is due within 30 days of enrollment start date or the beginning of the following month.
How to enroll?
Complete and SUBMIT this google form with all the required details duly filled in.
How to terminate or cancel out of the program?
Send an email request to pujabooking@gnht.org, 30-days ahead of your normal payment date.
You are responsible to terminate [FILL ME] if that is what you had setup for automatic payments.
We cannot prorate any mid-month cancellations after payment has been made, so plan accordingly.
