General Disclaimer
I, the undersigned parent or legal guardian of the child (“Participant”), understand and acknowledge that participation in the Greater Nevada Hindu Temple (GNHT)’s Camp involves physical activity, arts and crafts, yoga exercises, movement games, the use of craft materials, outdoor or indoor play, and other activities that may involve inherent risks of injury. I voluntarily agree to allow my child to participate in all camp activities and programs.I acknowledge and assume full responsibility for any and all risks, injuries, or damages that may be sustained by my child as a result of participation. I hereby release, waive, discharge, and hold harmless:
At GNHT Camps, all camp organizers, teachers, volunteers, and assistants; any partnering organizations/community members; the owners and staff of the GNHT and all related parties to this camp from any and all liability, claims, demands, actions, or causes of action arising out of or related to, but not limited to any loss, damage, illness, or injury that may be sustained by my child while participating in the camp. This includes, but is not limited to accidental injury, allergic reactions, property loss or damage, injuries during physical activities, yoga, games, or movement, injuries from craft tools (scissors, glue, art materials, etc.)
In the event of a medical emergency, I authorize the camp organizers, teachers, or volunteers to provide basic first aid, contact emergency medical services (EMS), consent to treatment on behalf of my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred on behalf of my child.
I understand that the GNHT Camp promotes respect, safety, and positive behavior.
I agree that: My child must follow instructions from teachers and volunteers; Unsafe behavior may result in removal from an activity or, if necessary, the camp; Refunds are not guaranteed in cases of behavioral dismissal.
The camp may provide snacks such as chips, fruit, juice boxes, and candy. I agree to communicate allergies clearly in this form and understand that: The camp organizers and volunteers will make reasonable efforts to avoid allergens. The camp cannot guarantee an allergen-free environment.
